On the May 7 return of Jersey Shore: Family Vacation, Pivarnick told Sammi "Sweetheart" Giancola and Nicole "Snooki" Polizzi that she was single and having unprotected sex with multiple men. She added, "I would definitely welcome a pregnancy in my life right now," before saying, "It’s something I always wanted in my life. I always wanted to be a mom, and I want to start my own family because I don’t have my own family."
That same episode said she had purchased donor sperm and had begun the early consultation stages for IVF. The episode was filmed in March 2025, so the storyline was already well underway before it reached viewers.

The March 2025 episode ended with Pivarnick taking a pregnancy test in the bathroom of Mike "The Situation" Sorrentino’s daughter’s first birthday party. A previous preview had already shown her holding up a positive pregnancy test, which pushed the story into spoiler territory before the season aired.
By September 2025, she posted photos on her Instagram Story that appeared to be taken at a doctor’s office or fertility clinic. She also later posted that she became pregnant at some point, though she did not say what happened with that pregnancy, and she said Vinny Tortorella was not the father.
