The Trump administration said Trump ACA refund checks of $500 will go to nearly 1 million Americans starting in October 2026. The payments apply only to people who bought an Affordable Care Act plan through HealthCare.gov and do not receive premium tax credits or other premium assistance.
That narrows the program to a small slice of marketplace customers. The White House said the checks come from user fees collected through the federal insurance marketplace during the Biden administration, and those fees were paid by insurers and built into premiums.
HealthCare.gov and 30 states
The refund program covers people in 30 states that use the federal exchange: Alabama, Alaska, Arizona, Arkansas, Delaware, Florida, Hawaii, Indiana, Iowa, Kansas, Louisiana, Michigan, Mississippi, Missouri, Montana, Nebraska, New Hampshire, North Carolina, North Dakota, Ohio, Oklahoma, Oregon, South Carolina, South Dakota, Tennessee, Texas, Utah, West Virginia, Wisconsin and Wyoming. The estimate that about 1 million people will receive a check leaves most of the roughly 19 million Americans enrolled in ACA marketplace coverage outside the payment pool.
Because the payment is flat, the $500 amount does not mean every recipient overpaid exactly that sum. The checks are described as a distribution from an aggregate surplus, not a one-to-one refund tied to each person’s documented premium charges.
The White House payment formula
The White House has said the money comes from excessive user fees collected through HealthCare.gov. Marketplace fees are calculated as a percentage of premiums, so the administration’s flat payment uses a pooled amount rather than a person-by-person calculation.
That leaves the practical question for consumers: how will they know they qualify, and how will the government tell them? Officials have not publicly detailed how recipients will be notified, how consumers can verify eligibility, or whether there will be a process for challenging exclusions.
October 2026 timing
The first checks are scheduled to begin going out in October 2026. For households that bought coverage through HealthCare.gov and do not get premium assistance, the next step is straightforward: wait for the notification process the administration has not yet explained.
