Named the Advancing Chronic Care with Effective, Scalable Solutions Model, this payment structure emphasizes outcome alignment. Providers will be compensated based on their ability to achieve specific, measurable health outcomes. The model’s design seeks to incentivize effective management of the following chronic conditions:
- High blood pressure
- Diabetes
- Musculoskeletal pain
- Depression
Compensation Structure
Under this new model, CMS will issue fixed payments to participating providers for patients with qualifying conditions. The complete payment will be contingent upon demonstrating successful health outcomes.
Key Dates and Timeline
The application process for this 10-year voluntary model will open on January 12, 2026. Interested providers must submit their applications by the initial deadline of April 1, 2026. The program is scheduled to commence on July 1, 2026.
