The recent negotiations between the Quebec government and family doctors have revealed significant details about the new agreement. The government, led by François Legault and Christian Dubé, aims to reshape the remuneration structure for family physicians, marking what may be the most substantial change in over 55 years, since the establishment of the health insurance system.
Key Points of the Agreement
After a six-day negotiation blitz concluded, the specifics of the agreement remain somewhat unclear without access to the finalized contract. However, several vital aspects have been highlighted:
- Implementation Date: The newly amended law will come into force on February 28, 2026, instead of the previously scheduled January 1, 2026.
- Compensation Model: The government has scrapped the punitive bonus structure proposed, initially set to reduce payments by 10% unless performance targets were met.
- Conditional Pay Increase: Instead, family doctors may see a conditional increase in their remuneration, dependent on achieving specified targets.
- Adjustment of Performance Targets: Performance targets, previously set at a rate of 83% for patient management, may be adjusted. For example, the push to raise this rate to 95% for complex patients might be downgraded to a more achievable level for 2027.
Performance Metrics Readjusted

Approximately 10,000 members will have seven days to vote on the new agreement. The results are expected to be announced publicly by the morning of December 19, 2026.
